Our mission is grounded in stewardship and partnership.

We stay closely connected to the organizations and Coaches who bring CTI® to life, offering guidance, learning opportunities, and tools as they navigate the realities of this work. This ongoing connection matters because CTI is a practice that develops through experience and curiosity, and we believe in the power of peer-to-peer learning to strengthen that practice over time.

Our commitment to the CTI® network is a commitment to the health of the communities it serves.

When CTI Program Providers and Coaches are supported, the model is delivered with greater consistency and depth - allowing individuals, families, and care partners to build the skills needed to manage their health and navigate complex systems with greater confidence.

We remain actively engaged beyond training because sustaining this work requires more than learning the steps. Through direct, ongoing support, we help organizations implement, adapt, and grow their programs ensuring they are not doing this work alone.

Because when the network is strong, the impact reaches further.
