Thank you for your interest in the Care Transitions Intervention® Coach Training. We have a very experiential and hands-on virtual training conjunction with a self-paced pre-training course. Read about the process and components below. We offer exclusive, in-person, group trainings when it's responsible to travel.

Pre-Training Course: Three (3) hours in our Learning Management System. This begins the initial understanding of what and how the model works. The pre-training course is administered two (2) weeks prior to the virtual classroom training.
Virtual Classroom Training: 16-hour live virtual classroom training (held in various, multi-day formats). This is where the CTI pre-training course concepts become reality and the trainees apply and practice Transition Coach skills and techniques while diving deeper into the models process. The training is very hands on and should be consider “on the job training” for attending staff.
Post-Training expectations: New Transitions Coaches® are most successful when they begin coaching clients within the first 6-weeks post-training. They also benefit from observing other Coaches and having their own encounters observed to receive focused feedback from CTI team member (if possible).
Note: Prior to CTI Coach training, all organizations must go through the Readiness Assessment with the CTI HQ Team to think strategically in order to execute the model implementation appropriately. We’ll support project leads to complete a Readiness Assessment Tool. It’s designed to help program providers think through crucial elements (workflow, staffing, metrics, sustainability, etc.) of the CTI and adhere to model fidelity as a way to ensure success.
