Thank you for your interest in the Care Transitions Intervention® Coach Training. We have a very experiential and hands-on virtual training that consists of a four (4) hour self-paced pre-training course & sixteen (16) hour live virtual classroom course. Read about the process and components below. We offer exclusive, in-person, trainings for 12+ students.

Pre-Training Course: Four (4) hours in our Learning Management System. This begins the initial understanding of what and how the model works. The pre-training course is administered two (2) weeks prior to the virtual classroom training.
Virtual Classroom Training: 16-hour live virtual classroom training (held in various, multi-day formats). This is where the CTI pre-training course concepts become reality and the students apply and practice Transition Coach® techniques while diving deeper into the models process. The training is very hands on and should be consider “on the job training” for attending staff.
Post-Training expectations: New Transitions Coaches® are most successful when they begin coaching clients within the first 6-weeks post-training. Join our robust network of CTI Program Providers by attending the monthly CTI Community Learning call or other advanced learning opportunities.
Note: Prior to CTI Coach training, all organizations must go through the Readiness Assessment with the CTI National Office. It’s designed to help think through crucial elements of the CTI (workflow, staffing, metrics, sustainability, etc.) and adhere to model fidelity as a way to ensure success.
