Our interactive, practice-based course prepares professionals to support individuals navigating transitions of care from facility to home after a medical event - bringing a truly person-centered coaching approach to life.
Learning by Doing
Rather than simply learning about the Care Transitions Intervention® (CTI) model, students experience it! Our three-stage training experience moves from introductory knowledge to hands-on practice and ultimately to applying new coaching skills with their clients.
Build an initial understanding of the CTI model, its guiding principles, and the role of the Transition Coach. Through videos, readings, quizzes, and modeled coaching interactions, participants prepare for the live classroom.
Highly experiential training brings the CTI model to life. Building on the pre-training, students dive deeper into the intervention proven philosophy while actively practicing coaching skills and communication techniques.
Through case studies, interactive exercises, and role plays, students learn by doing; receiving direct feedback from instructors and peers in a supportive learning environment.
Real-world application is necessary to strengthen skills and build confidence through experience. Each interaction is a chance to continue developing those coaching muscles.
Transitions Coaches have ongoing support locally and through the CTI National Office. Live and on-demand education, practical resources, and live monthly learning calls connecting to the CTI community.
